The coach will decide who he thinks should play where for your entire lineup. Its actually pretty effective I think if you are not concerned with minutes or training.

the coach does not know when the game is won -- if you are up by 20 at the end of three periods, the coach will still play the best players available.

I've been using blank lineups and LCD for my Utopia private league games. It's a pretty nice way to see how the computer stacks up your team against an opponent. Hassan is correct, there is no garbage time (not that it matters with my terrible Utopia team). The coach keeps the most effective lineup in as much as it can.
